 Pay Automatically Instructions

Step 1. Instructions for completing the form:

Contact your financial institution to ensure you are eligible for automated payments.
Complete the EFT authorization form using the steps below.
[National City Mortgage Loan Number] - Input your mortgage loan number (10 digits).
[Your Name] - Input your name here.
[Name of Primary Mortgage Account Holder] - Input the name, which appears on your mortgage account.
[Daytime Phone Number] - Input your daytime phone number. Do not use hyphens.
[Your Financial Institution's Name] - Input the name of your financial institution.
[Your Financial Institution's Phone Number] - Input the phone number of your financial institution. Do not use hyphens.
[Deduct my payment on the following day of each month] - Select desired day you would like for your payment to be automatically paid from your checking or savings account (if no date is selected, your payment will be deducted on the due date).
[Additional Principal Amount] - Optional / Enter additional amount (if any) you want applied to your principal balance each month.
[Account Type] - Please select the account type you would like your automatic payments to be deducted from. Your choices are checking or savings.
[Financial Institution's ABA Routing Number] - Input the ABA Routing Number of your financial institution. These numbers are located at the bottom of your check within the scan line. Use the first 9 numbers from the left. If this number begins with anything other than 0, 1, 2 or 3, please check with your financial institution for the correct ABA number.
[Financial Institution's Account Number] - Input your account number. These numbers are located at the bottom of your check within the scan line, usually in the middle. Please contact your financial institution for questions regarding your account number.
[Date] - Input today's date (mm/dd/yyyy format).
After all information is entered, print form.


Step 2. Complete Authorization Form

Step 3. Mail completed & signed form to:

It is important that you return the authorization form, along with a voided check or a savings deposit slip from your account. We may not be able to process your enrollment form without these documents. Please forward a separate authorization form for each loan.
